Anastrozole Tablets are an antineoplastic drug used to treat certain types of breast cancer in postmenopausal women. It helps slow the growth of hormone-dependent tumors that need estrogen to grow. It is most often prescribed after cancer has progressed and after treatment with tamoxifen.

Anastrozole is a first-line therapy for advanced or metastatic cases of hormone-sensitive breast cancer. Those who are prescribed anastrozole typically will have undergone surgery, radiotherapy, and occasionally chemotherapy. This antiestrogen agent may also prescribed to postmenopausal women who have an increased risk of developing breast cancer. Though breast cancer is typically rare in men, this medication is sometimes used to treat breast cancer in males.

Though it is primarily used to treat breast cancer, anastrozole is occasionally used as a fertility treatment for women with polycystic ovary syndrome (PCOS).

Anastrozole is on the Essential Medicine List, which is published by the World Health Organization.

Anastrozole is classified as a nonsteroidal aromatase inhibitor. It works by preventing the conversion of androstenedione to estrone and estradiol in the peripheral tissues. This effect then decreases the amount of estrogen made by the body, leading to less of the hormone available for the tumor to utilize.

Anastrozole Tablets are manufactured with a dosage of 1mg. Each box contains 10 blisters, each containing 10 tablets.

Uses

What is Anastrozole used for?

It is used to treat breast cancer in postmenopausal women. It is indicated for:

  • hormone receptor-positive, advanced breast cancer
  • adjuvant treatment of hormone receptor-positive, early invasive breast cancer
  • adjuvant treatment of hormone receptor-positive, early invasive breast cancer after 2-3 years of tamoxifen therapy

It is occasionally used for breast cancer in men and breast cancer in premenopausal women.

How are Anastrozole Tablets used?

This medication is manufactured to be taken orally. Anastrozole Tablets may be taken with or without food. It is recommended to take this medication with a full glass of water. Do not crush or chew the tablets.

How should Anastrozole Tablets be stored?

It is recommended to keep the tablets in their packaging until use. They should be stored at room temperature in a location free of moisture and heat. Any expired medicines should be discarded in accordance with local regulations.

This medication should be kept out of the reach of children and pets.

What dose should be taken?

Adult Dosing Dosage may vary based on different medical indications:

  • For first-line treatment of breast cancer (hormone receptor-positive or hormone receptor unknown; advanced, progressive metastatic) in postmenopausal women, the usual dose is 1mg, taken once per day. Treatment should continue until tumor progression.
  • For second-line treatment of breast cancer (advanced, disease progression following tamoxifen therapy) in postmenopausal women, the usual dose is 1mg, taken once per day. Treatment should continue until tumor progression. It is important to note that typically patients who did not respond to tamoxifen therapy do not often respond to anastrozole.
  • For postmenopausal breast cancer (adjuvant treatment for hormone receptor-positive early disease), the usual dose is 1mg, taken once per day. Treatment should be for 5 years.
  • For ovulation induction, the usual dose is 1-5mg, taken once a day for 5 days. Treatment should start on the third day of the menstrual cycle.

Renal Dosing Adjustments are not defined for this medication.

Hepatic Dosing No adjustment is needed for mild to moderate impairment or stable cirrhosis. Dosing adjustments have not been defined for severe impairment.

Pediatric Dosing For precocious puberty in male children over 2 years old, the usual dose is 1mg, taken once per day. Please note that this medication is not approved for use in children by the FDA.

Renal and hepatic dosing for pediatric patients has not been defined. The adult dosing guidance should be considered for children suffering from renal or hepatic impairment.

Refer to a doctor or pharmacist for guidelines on dosage. Do not exceed what they advise.

What happens if a dose is missed?

The missed dose should be taken as soon as possible. If it is almost time for the next dose, the missed dose should be skipped. Do not take two doses at the same time.

Who can use Anastrozole?

Anastrozole tablets can be given to adults and children, but caution is advised for specific groups of patients.

Pregnant The use of anastrozole is contraindicated for pregnant women, though there are no available human data, animal studies have shown a risk of fetal toxicity and death. It may be necessary to perform a pregnancy test before beginning treatment with this drug. Pre-menopausal women should use an effective form of birth control and continue its use for at least 3 weeks after discontinuing treatment with anastrozole.

Breastfeeding Breastfeeding should be avoided during treatment with anastrozole and for at least 15 days afterward. It is possible that anastrozole is excreted in milk. There is no human data available to properly assess the risk of harm to the infant. Animal studies have shown a risk of tumorigenicity associated with anastrozole.

Children Though studies have not demonstrated specific problems for this population, the use of anastrozole is not recommended in pediatric patients. This medication should not be used in combination with growth hormone treatment.

Geriatric Studies have not shown specific problems that would affect the use of this medication in elderly patients. It should be noted that post-menopausal and elderly patients have an increased risk of fractures and osteoporosis, as this medication may decrease mineral bone density.

Other warnings

Anastrozole may impair fertility in woman in their reproductive years.

For women with pre-existing ischemic heart disease, anastrozole should only be used if the benefits outweigh the risks. There is an increased incidence of ischemic cardiovascular events in this population.

Women with preexisting osteopenia have an increased risk of developing osteoporosis.

Anastrozole is associated with decreased mineral density of the lumbar spine and hip bone, which can lead to an increased risk of fractures and the development of osteoporosis. It is advised to consider the concurrent treatment with bisphosphonates to reduce the risk of fractures.

It is recommended to monitor serum cholesterol during treatment with this drug. For patients with hyperlipidemia, this drug should be used cautiously. Cholesterol levels should be monitored and managed for patients with elevated LDL.

Side Effects

As with all pharmaceuticals, some unwanted effects can occur from the use of Anastrozole Tablets.

Common side effects include, but may not be limited to:

  • numbness, tingling, or a tickling feeling
  • hot flashes
  • weakness
  • joint pain
  • nausea or vomiting
  • fatigue
  • pain
  • arthritis or arthralgia
  • mood disturbances, including depression

Side effects are expected to improve after a few months of treatment.

Seek medical attention if the following develop:

  • shortness of breath or rapid weight gain
  • symptoms of a bone fracture
  • numbness, prickling, pain, or weakness in the hands or wrists
  • signs of a stroke

Precautions

Do NOT use Anastrozole Tablets if:

  • You are allergic or hypersensitive to any of the ingredients.
  • You are a male or a child.
  • You are currently taking tamoxifen.

This medication may not be suitable for people with certain conditions, including:

  • heart problems (e.g. heart attack, angina)
  • coronary artery disease
  • high cholesterol (hypercholesterolemia)
  • osteoporosis
  • liver disease

While it is rare for a postmenopausal woman to become pregnant, it is important to note that this medication could harm a fetus if pregnancy occurs.. Pre-menopausal women should use effective birth control measures for the duration of treatment and at least 3 weeks after the end of treatment. Do not breastfeed while taking Anastrozole. Consult a doctor or healthcare professional before treatment.

Many drugs are known to interact with anastrozole, including other estrogen medications such as hormone replacement therapy or estrogen cream.

Some formulations for this tablet may contain lactose. These formulations should not be taken by patients with hereditary galactose intolerance, lactase deficiency, or glucose-galactose malabsorption.

Anastrozole is not expected to interfere with the ability to drive or operate machinery. It should be noted that there have been some reports of tiredness, so patients should exercise caution before performing tasks requiring mental focus.

Anastrozole may affect blood pressure, cholesterol levels, and bone density. It is likely that regular monitoring and blood tests will be necessary while undergoing treatment with this medicine.

Anastrozole 1mg: Why the most common dose?

Anastrozole 1mg is the standard dose for treating hormone receptor-positive breast cancer in postmenopausal women. This dose effectively reduces estrogen levels, helping to slow the growth of cancer cells. It is well-tolerated and provides a reliable option for long-term treatment.

Anastrozole in early breast cancer

Anastrozole is a nonsteroidal selective aromatase inhibitor, that has recently been approved in the US and several other countries for the adjuvant treatment of postmenopausal women with hormone receptor-positive early breast cancer.

Anastrozole 1mg has been proven to be significantly more effective than tamoxifen 20mg or combined treatment (17 and 19% relative risk reduction) for disease-free survival in postmenopausal women with early breast cancer. It is also more effective than tamoxifen for time to tumor recurrence and the odds of a primary contralateral tumor as a first event.

Compared to tamoxifen, anastrozole was linked to a significantly lower incidence of vaginal bleeding, vaginal discharge, hot flashes, endometrial cancer, ischemic cerebrovascular events, venous thromboembolic events, and deep vein thrombosis, including pulmonary embolism.
