The study investigated the efficacy of 0.5mg melatonin combined with behavioral sleep-wake scheduling in improving sleep initiation for individuals diagnosed with Delayed Sleep-Wake Phase Disorder (DSWPD) who exhibited a delayed endogenous melatonin rhythm relative to their desired bedtime (DBT).
A total of 307 participants were included, with 116 receiving melatonin treatment. Compared to the placebo group, those receiving melatonin experienced sleep onset 34 minutes earlier. Melatonin treatment also resulted in increased sleep efficiency at the first attempt to sleep (SE T1), as well as reductions in PROMIS sleep-related impairment, PROMIS sleep disturbance, insomnia severity, and functional disability. More patients demonstrated more than minimal clinician-rated improvement with melatonin treatment (52.8%) compared to placebo (24.0%) (P < 0.05).
The study findings suggest that administering melatonin 1 hour before the desired bedtime, in conjunction with behavioral sleep-wake scheduling, effectively improves both objective and subjective measures of sleep disturbances and related impairments in individuals with DSWPD who exhibit a delayed circadian phase relative to DBT. These improvements are primarily due to the sleep-promoting effects of melatonin, complemented by behavioral sleep-wake scheduling strategies.
