What are Gas Relief Capsules?
Active Ingredients: Simethicone
Gas Relief Capsules (Simethicone) are used to relieve symptoms caused by excessive gas (flatulence) in the stomach and intestines. It helps relieve symptoms like painful pressure, fullness, and bloating. Simethicone is also used to help relieve symptoms associated with irritable bowel syndrome (IBS). Simethicone has been in use since the 1940s but was granted FDA approval in 1952.
Simethicone works quickly, typically within 30 minutes, and is considered safe for use by babies, children, and adults.
Simethicone is a compound formed from a mixture of polydimethylsiloxane and hydrated silica gel. It is classified as an antiflatulent or a gastrointestinal agent. Simethicone works by changing the surface pressure of the gas bubbles in the digestive system. It decreases the surface tension. This action causes them to clump together and pass through the system more easily. It leads to their expulsion. Its duration of action is short, and it is given as needed. It has a wide therapeutic index because it is not systemically absorbed. This drug allows the gas to exit the body more easily.

Uses
What is Gas Relief used for?
It is used to treat trapped gas (wind) in the digestive system. Simethicone is also used in the treatment of irritable bowel syndrome (IBS).
How are Gas Relief Capsules used?
This medication is manufactured to be taken orally.
What dose should be taken?
For wind, the usual dose is 140mg, as needed, 3-4 times per day. It is best to take this medication after meals and before bedtime.

Why do humans take Gas Relief Capsules?
There are 3 main reasons for gas in the gastrointestinal system, including:
- excessive belching
- distention (bloating)
- excessive flatus
Excessive belching or eructation is due to the swallowed air or gas from carbonated beverages. Aerophagia during eating can also lead to increased gas presence. Eating, smoking, and anxiety are all conditions that might lead to gastroesophageal disorders. Supragastric belching can also lead to additional gas presence. It means expelling rapid entry of air into the esophagus.
Distention or abdominal bloating is due to aerophagia, non-ulcer dyspepsia, gastroparesis, irritable bowel syndrome, as well as organic disorders.
Intestinal bacteria are the primary source of flatus, a metabolic byproduct, with minimal contribution from swallowed air or the back-diffusion of gases, primarily nitrogen, from the bloodstream. Bacterial metabolism within the intestines produces substantial amounts of hydrogen, methane, and carbon dioxide.
All these conditions can be treated with the active ingredient from the Gas Relief Capsules. The treatment process is due to the agent Simethicone which breaks up small bubbles.
What are other indications of Gas Relief Capsules in humans?
Simethicone is the active ingredient of this drug, and it is commonly included as an adjunct in bowel preparation before colonoscopy, but there is currently no unanimous consensus on whether it should be a standard recommendation in bowel preparation protocols. Simethicone significantly increased the odds of adequate bowel preparation for single dosing but not for split dosing. Regarding patient tolerability, the addition of simethicone was linked to significantly lower odds of experiencing abdominal bloating, with no notable effects on nausea, vomiting, and abdominal pain. In terms of adenoma detection rate (ADR), although simethicone does not show a significant overall effect, studies show that single-dosing bowel preparations demonstrated a significant improvement in ADR.
Recognizing the critical role of bowel preparation quality in the success of colonoscopy, both diagnostically and therapeutically, is critical. Poor bowel preparation has been associated with increased adverse events and a reduced adenoma detection rate. Simethicone's value as an adjunct in bowel preparation stems from its detergent-like function, reducing the formation of bubbles that could hinder the visualization of the mucosa.
Are Gas Relief Capsules enough for treating some health issues?
It depends on the current health condition of the patient. The doctor should prescribe the recommended dosage for Gas Relief Capsules depending on the current health condition, patient’s age, and weight. Depending on the reason for excessive gas production, some patients might need additional treatment with anticholinergics, antacids in cases of dyspepsia and postprandial upper abdominal fullness, and additionally low doses of tricyclic antidepressants. Activated charcoal can be occasionally effective in reducing gas and mitigating unpleasant odors, though it has the drawback of staining clothing and the oral mucosa. Specialized charcoal-lined undergarments are available to address this issue. Probiotics might offer relief from bloating and flatulence by influencing intestinal bacterial flora, although data in this realm are currently limited. For individuals with confirmed bacterial overgrowth, antibiotics can be a useful intervention.

Are Gas Relief Capsules enough for treating IBS?

Is Simethicone safe for geriatric patients?
Simethicone is often used in older patients for colonoscopy preparation. However, there are not many studies that can confirm the safety of the drug. The doctor should decide about the prescription for older patients.
Can pregnant women use Gas Relief Capsules?
Simethicone, the active ingredient of the drug, does not enter the bloodstream, and it is only active in the gut. Pregnant women can take this medication but always following the recommendations of the doctor and gynecologist.
Is Simethicone safe during breastfeeding?
Yes, this drug does not enter the bloodstream and also does not get into the breast milk.
How should Gas Relief Capsules be stored?
This drug should be stored at room temperature, away from light and moisture, and out of reach of children. Discard any unused medication after the expiration date or if it appears cloudy or discolored.
Side Effects

Precautions
Do NOT use Gas Relief Capsules if:
- You are allergic to Simethicone or any of the other ingredients.
- You are undergoing treatment for a thyroid problem.
Before treatment, consult your doctor regarding any medications you are taking to address potential drug interactions.
Patients should inform their doctors if they are taking prescription or nonprescription medications. Other drugs in combination with this drug can increase the risk for serious side effects. Patients should not start or stop the medication or the dosage without a doctor’s approval.
Simethicone is not considered harmful for use during pregnancy and while breastfeeding but patients should inform their doctor if they are pregnant, planning to become pregnant, or are breastfeeding.
If patients are treating some chronic problem and they do not see any further improvements, they should notify their doctor.
Simethicone should be used on a regular schedule. If a dose is missed, the patient should skip to the next scheduled dose. If patients miss a dose, they should not take double doses.
This drug should be kept out of reach of children.

Systematic review and meta-analysis of randomized controlled trials of Simethicone for gastrointestinal endoscopic visibility
The main objective of this study was to evaluate the supplemental use of Simethicone in endoscopy including capsule endoscopy (CE), colonoscopy, and esophagogastroduodenoscopy. This is a systematic review and meta-analysis of randomized controlled studies on the use of Simethicone for endoscopy. During the study, the following endpoints were examined: small bowel visualization quality (SBVQ), completion rate, gastric transit time, small bowel transit time, diagnostic yield, the efficacy of bowel preparation, degree of air bubbles, and duration time.
The results from 13 studies were included and 4 studies compared the purgative or fasting plus Simethicone with purgative or fasting alone for capsule endoscopy. Patients who received supplemental simethicone before colonoscopy had a better SBVQ ([odds ratio] OR = 2.84, 95% CI: 1.74–4.65, p = 0.00). Seven studies compared purgative plus Simethicone with purgative alone for colonoscopy. Patients who received supplemental simethicone had decreased air bubbles (OR = 39.32, 95% CI: 11.38–135.86, p = 0.00).
The conclusion of the study is that supplemental simethicone improves the SBVQ and decreases air bubbles in the colonic lumen.
