Cold Syrup (Dextromethorphan HBr, Guaifenesin, Phenylephrine HCl, Paracetamol) is a combination drug used for treating the common cold, flu, or allergies. This medicine helps relieve symptoms such as stuffy nose, coughing, congestion, and sneezing. This formulation is not used for coughing caused by respiratory conditions such as asthma or emphysema.

This product is available in two formulations: Cold&Flu Relief™ and Cold&Flu Relief MAX™. Both formulas contain the following three active ingredients:

  • Dextromethorphan HBr is a cough suppressant that is classified as a morphinan. It is typically used for its antitussive properties. It works by interfering with the cough reflex.
  • Guaifenesin is an expectorant that helps thin the mucus and phlegm in the lungs.
  • Phenylephrine HCl is a decongestant that acts as a vasoconstrictor. It narrows blood vessels, which helps reduce swelling, itching, and discomfort.

Cold&Flu Relief MAX™ also includes the pain reliever and antipyretic, Paracetamol (acetaminophen), which helps reduce fever and relieve headache and pain.

Paracetamol, included in the MAX™ version, is a widely used in reducing fever and alleviating pain associated with cold and flu symptoms. Due to its presence in many over-the-counter and prescription drugs, caution must be exercised to avoid exceeding the recommended daily dosage. Excessive intake can lead to liver damage. This is crucial for individuals with pre-existing liver conditions or those who consume alcohol regularly.

The combination of Dextromethorphan HBr and Guaifenesin addresses both the dry cough and the mucus production aspects of cold and flu symptoms. Dextromethorphan acts centrally in the brain to suppress the cough reflex, while Guaifenesin's role as an expectorant assists in loosening and thinning respiratory tract secretions to facilitate a more productive cough. Side Effects

  • little or no urination
  • changes in heart rate (bradycardia)

The formulation of Cold Syrup containing Paracetamol warrants caution, particularly regarding liver health. Overdosing on Paracetamol can cause acute liver failure, which may not present immediate symptoms. Users must follow the recommended dosage and avoid combining it with other Paracetamol-containing products.

Patients should also be aware of the risk of rebound congestion with prolonged use of decongestants like Phenylephrine, which can lead to chronic nasal congestion if used for more than a few days consecutively. This effect, known as rhinitis medicamentosa, can create a cycle of dependency on nasal decongestants.

Phenylephrine may also worsen certain conditions such as glaucoma, hyperthyroidism, and hypertension. Its vasoconstrictive properties might lead to elevated blood pressure and exacerbate these conditions.

Precautions

Do NOT use Cold Syrup if:

  • You are allergic to any of the other ingredients.
  • You have used an MAOI within the past two weeks.

This product should not be used with other drugs that cause drowsiness, sedation, or respiratory issues, including opioids and anxiety medicines.

The ingredients in this medicine may not be suitable for people with the following conditions:

  • heart disease
  • hypertension
  • thyroid disorders
  • glaucoma
  • enlarged prostate
  • seizure disorders
  • adrenal gland tumor
  • a wet cough or cough caused by a breathing disorder
  • pregnant or breastfeeding

Do NOT use Cold Syrup to induce sleep in children. Drink more fluids while undergoing treatment with Cold Syrup to assist the expectorant action of Guaifenesin, helping to thin and loosen mucus in the airways. Patients with certain medical conditions, such as heart failure or kidney disease, should consult their doctor about appropriate fluid intake to avoid exacerbating their condition.

Consider the potential for overuse or misuse of Cold Syrup containing Dextromethorphan HBr. While effective as a cough suppressant, Dextromethorphan can be subject to abuse, leading to serious side effects such as severe drowsiness, increased heart rate, and altered mental states. Patients with a history of substance misuse should be particularly cautious.

Objective and self-reported evidence of dextromethorphan antitussive efficacy in children, aged 6–11 years, with acute cough due to the common cold

This study evaluated the antitussive effects of dextromethorphan hydrobromide (DXM) in children, and both objective and subjective measures were considered.

The study was designed as a multiple-dose, double-blind, placebo-controlled, randomized pilot clinical trial involving children aged 6–11 years with cough due to the common cold. Eligible participants underwent a run-in period where their coughs were monitored after receiving a sweet syrup dose. They were then randomly assigned to receive either DXM or a placebo over a 4-day period. Coughs were recorded during the initial 24 hours.

Analysis of data from 128 patients (67 patients DXM; 61 patients placebo) revealed that DXM reduced total coughs over 24 hours (the primary endpoint) and daytime cough frequency by 21.0% and 25.5%, respectively, compared to placebo.

The conclusion is that DXM has antitussive efficacy in children.
