Clobetasol Propionate Cream is a topical steroid drug used to treat mild to severe inflammation and itching. It is indicated for the treatment of conditions like plaque psoriasis, atopic dermatitis, and steroid-responsive skin conditions in adults and children over 12. This medication should only be applied topically to the skin.

Clobetasol Propionate is a potent glucocorticoid steroid with anti-inflammatory, antipruritic, and vasoconstrictive properties. This medication is a synthetic corticosteroid and it is intended for dermatologic use. It is an analog of prednisolone and has a high degree of glucocorticoid activity and a low mineralocorticoid activity.

The mechanism of this product is unclear, but it is thought to act by induction of phospholipase A2 inhibitory proteins. These proteins control the biosynthesis of potent mediators of inflammation like prostaglandins and leukotrienes by inhibiting the release of their common precursor, arachidonic acid. The absorption of this product is determined by the vehicle and the integrity of the epidermal barrier. This drug is absorbed well in normal intact skin.

Clobetasol Propionate is also available as an ointment, spray, or lotion.

What is Clobetasol Dipropionate used for?

It is used to treat moderate to severe inflammation and itching associated with conditions like eczema and psoriasis.

How is Clobetasol Cream used?

This medication is manufactured as a sterile cream, which is intended for topical administration directly to the skin. Always wash your hands with soap and water before and after using this medication.

What dose should be applied?

A small amount of cream should be applied to the afflicted area. Only a thin layer of this cream should be applied to the affected skin area twice daily. It should be rubbed gently and completely.

The general recommended dose for this cream should be limited to 2 consecutive weeks and the weekly dose should not exceed 50g/week.

Adult Dosing The usual recommended dose for adults with skin issues is 50g/week for a maximum of 2 weeks of treatment.

Geriatric Dosing This cream is safe for geriatric patients and the safety does not differ from that in younger patients. There is no need for dosage adjustment for these patients.

Pediatric Dosing Clobetasol is not recommended for patients younger than 12 years.

There is no data available about dosing in patients with hepatic or renal impairment.

Who can use Clobetasol Propionate?

Clobetasol Propionate can be used for the relief of the inflammatory and pruritic manifestations of corticosteroid-responsive dermatoses. Special considerations regarding the usage of this drug should be made before applying it to some groups of patients.

Pregnant According to animal studies, corticosteroids might be considered teratogenic after dermal applications. Some of the most common abnormalities are cleft palate and skeletal abnormalities.

There are no adequate and well-controlled studies about the safety and the teratogenic potential of this drug in pregnant women. It can be used during pregnancy only if the benefits outweigh the potential risks to the fetus.

Nursing Corticosteroids that are given systematically can appear in human milk, however, it is not known if topical corticosteroids appear in human milk. This cream should be given to nursing women with caution.

Pediatric This drug should be applied to children only if they are above 12 years of age. There is a risk of hypothalamic-pituitary-adrenal HPA axis suppression and Cushing’s syndrome in pediatric patients treated with topical corticosteroids. These patients are also at a higher risk of adrenal insufficiency after drug withdrawal.

Geriatric Elderly people are at increased risk of many health issues and they need to take most of the drugs with caution. These patients should consult with a doctor before taking this cream.

Other warnings

Hypothalamic‐pituitary‐adrenal (HPA) axis suppression might occur in patients who are treated extensively with this drug. If this occurs, the drug should be withdrawn.

Pediatric patients should not be treated with this cream because they are susceptible to systemic toxicity.

In cases of irritation, the treatment should be discontinued and appropriate therapy should be given.

If concomitant skin infections occur, antifungal or antibacterial drugs should be given to the patient based on the cause of the skin infection.

Side Effects

Precautions

Evaluation of the Efficacy and Safety of Clobetasol Propionate Spray in the Treatment of Plaque-Type Psoriasis

The main objective of this study is to evaluate the safety and efficacy of clobetasol propionate in the treatment of moderate to severe plaque-type psoriasis. This is a multicenter, randomized, double-blinded, vehicle-controlled, parallel-group, comparative study.

All the participants received either a clobetasol propionate spray of 0.05% or a vehicle spray twice daily for 4 weeks. These patients were followed by a 4-week treatment-free follow-up. The patients were evaluated for scaling, erythema, plaque elevation, pruritus, and overall disease severity. The success rates were evaluated, and the results were significantly in favor of clobetasol propionate (P ≤ .001).

Extending the treatment duration by an additional 2 weeks from weeks 2 to 4 increased the number of cured subjects from 2% to 25%. Even at week 8, which was 4 weeks posttreatment, clobetasol propionate remained more effective compared to other treatments (P < .001). There were no serious adverse events due to clobetasole propionate. The most common treatment-related side effect was mild burning or stinging at the application site, with similar occurrence and intensity in both the active and vehicle groups. There were no cases of skin thinning, visible blood vessels, hair follicle inflammation, or disruption of the body's hormone regulation system.

The conclusion of this study is that clobetasol propionate spray 0.05% twice daily for 4 weeks is effective and safe in reducing scaling, redness, plaque elevation, and overall severity of the condition. This product can maintain positive results for up to 4 weeks after treatment discontinuation.

A randomized, double-blind, placebo-controlled study of clobetasol propionate 0.05% foam in the treatment of nonscalp psoriasis

The goal of this study was to evaluate the safety and efficacy of clobetasol propionate foam in the treatment of psoriasis involving areas other than the scalp. It included 81 patients that had mild to moderate psoriasis.

During this research, erythema, scaling, and plaque thickness were evaluated. Safety was assessed from reported adverse events. After 2 weeks of treatment, clobetasol propionate foam showed superior improvement compared to placebo foam, as assessed by both investigators and subjects (P < 0.0005). The treated patients had improvements even 4 weeks after the treatment. The adverse effects were mostly mild to moderate application site reactions, and no participants discontinued treatment due to side effects.

This study indicates that clobetasol propionate foam is more effective than placebo for treating non-scalp psoriasis. Twice-daily application is well tolerated, and the compliance rates exceed 90% and have the potential to replace separate prescriptions for scalp and body treatment.
