Bisacodyl Tablets are a laxative drug used to relieve chronic or functional constipation. This medication is also used to manage neurogenic bowel dysfunction. Bisacodyl may also be used as part of bowel preparation before surgery, treatment, or medical examinations, such as colonoscopy.

Bisacodyl is typically recommended after other laxatives, such as bulk-forming laxatives or osmotic laxatives, have not worked. This medicine is available over-the-counter in many countries, though some stronger dosages may require a prescription in some places.

Bisacodyl is classified as a colonic laxative stimulant. It is a derivative of diphenylmethane. This medicine works directly on the colon to produce a bowel movement by stimulating enteric nerves. This activity causes a colonic mass movement (peristalsis). Bisacodyl also works by promoting the accumulation of water, which helps to soften stool.

What is Bisacodyl used for?

It is used to relieve constipation and manage neurogenic bowel dysfunction. It is also used as bowel preparation before some medical examinations, like a colonoscopy.

How should Bisacodyl Tablets be used?

This medication is intended to be taken orally. Bisacodyl Tablets should be taken at least an hour after having milk or antacids. Do not crush or chew the tablets.

It is recommended to drink more water than usual while taking laxatives such as Bisacodyl. Follow any dietary advice as outlined by a healthcare professional. Dietary changes, such as increased fiber intake, may also be helpful for constipation, with the exception of constipation induced by other drugs.

How should Bisacodyl Tablets be stored?

This medication should be kept in its original packaging until use. It should be stored in a cool, dry location that is below 30°C. Do not keep this product in direct sunlight or excessive heat. Always keep medications out of the reach of children and pets. Do not use this medication if it is past the printed expiry date on the packaging.

How long does it take for Bisacodyl Tablets to work?

When taken orally, this medication generally works within 6-12 hours.

Can Bisacodyl Tablets be used every day?

The long-term use of this medication is not recommended. Bisacodyl Tablets are a safe and effective short-term treatment of constipation. When it is used for longer, the body may become dependent on this medication for producing a bowel movement. It is advised to speak with a healthcare professional if constipation lasts longer than 5 days.

Can Bisacodyl Tablets be taken with other stomach medicines, such as antacids?

Bisacodyl Tablets should not be taken at the same time as certain medicines, such as antacids. Certain medications will alter the pH of the stomach, and this lower acidity will stop Bisacodyl from working. Do not take this medication with dairy products, such as milk, cheese, or yogurt, or with medicines like antacids or proton pump inhibitors (PPIs) such as omeprazole or pantoprazole. It is recommended to leave 1-2 hours between these medicines and Bisacodyl Tablets.

What dose should be taken?

Adult Dosing Recommended dosage may vary based on different medical conditions:

  • For constipation, the usual dose is 5-15mg, taken once per day. The maximum dosage is 30mg/day. The tablet should not be crushed or chewed. If complete evacuation is necessary, 30mg can be taken one time per day.
  • For bowel prep, the usual dose is 5-10mg taken once. It is typically given about 6 hours before polyethylene glycol-containing bowel prep solution.

Renal Dosing No adjustment is necessary for patients with renal impairment. Guidelines have not been defined for HD/PD.

Hepatic Dosing There are no defined guidelines for this population.

Pediatric Dosing For constipation, the usual doses are as follows:

  • adolescents and children over 12 years old: 5-15mg, taken at bedtime. The maximum dosage is 30mg per day. Do not use it for longer than one week
  • children from 6 to 12 years old: 5mg or 0.3mg/kg, taken either at bedtime or before breakfast. The maximum dosage is 30mg per day. Do not use it for longer than one week
  • children younger than 3 years old: oral tablets are not recommended for this group, as they may not be able to swallow tablets effectively

Renal Dosing for pediatrics Guidelines have not been established for children with renal impairment. It is advised to consider adult renal dosing guidelines.

Hepatic Dosing for pediatrics There are no defined guidelines for this population.

The dosage is based on medical condition, response to treatment, age, and weight. Refer to a doctor or pharmacist for guidelines on dosage. Do not exceed what they advise.

Who can use Bisacodyl Tablets?

Bisacodyl Tablets can be given to adults and children, but caution is advised for specific groups of patients.

Pregnant The use of this medicine can be used during pregnancy if the benefits outweigh the risks. There is no human data available to properly assess the risk of Bisacodyl use during pregnancy. The risk of fetal harm is not expected due to the drug’s minimal systemic absorption.

Breastfeeding The use of this medicine can be used during breastfeeding if the benefits outweigh the risks. There is no human data available to properly assess the risk of Bisacodyl while nursing. Bisacodyl does not pass into milk. The risk of infant harm is not expected due to the drug’s minimal systemic absorption by the woman. Adverse effects on milk production are also not expected.

Children Though Bisacodyl Tablets are not typically the first-line treatment for constipation in pediatric patients, it is considered safe for most children over 12 years old. Certain forms of Bisacodyl may be given to children younger than 12, but it should only be used under the guidance of a doctor or pharmacist.

Geriatric Bisacodyl is considered safe for this population, but the chronic use of laxatives such as Bisacodyl may cause dehydration, electrolyte imbalances, muscle weakness, and kidney issues. Fluid loss can be dangerous for some elderly patients, so it is advised to use this medicine under the guidance of a medical professional.

Other warnings

The long-term use or overuse of this drug may cause laxative dependence.

This medication is contraindicated for patients suffering from obstruction or severe impaction, those who display symptoms of appendicitis or acute surgical abdomen, and patients who have rectal bleeding or vomiting.

Extreme caution is advised for use in patients with gastroenteritis.

Side Effects

As with all pharmaceuticals, some unwanted effects can occur from the use of Bisacodyl Tablets.

Common side effects include, but may not be limited to:

  • abdominal pain or cramping
  • nausea
  • vomiting
  • weakness
  • dehydration
  • diarrhea
  • dizziness or fainting

Serious side effects may include:

  • allergic reactions
  • swelling, especially the face, tongue, and throat
  • persistent nausea, vomiting, or diarrhea
  • muscle cramps or weakness
  • irregular heartbeat
  • fainting
  • confusion

Precautions

Do NOT use Bisacodyl Tablets if:

  • You are allergic to any of the ingredients.
  • You are having abdominal surgery.
  • You cannot swallow without chewing.
  • You have severe stomach pain, constipation, or intestinal/rectal bleeding.

Due to possible drug interactions, consult with your doctor about any medications, supplements, or herbal products you are taking before your treatment. Adjusted dosages or additional monitoring may be necessary for certain medicines. Do not take this medication together with drugs that lower the acidity of the upper GI (stomach). Some drugs that are known to interact include certain bowel prep formulas (polyethylene glycol, sodium phosphate, sulfate), sodium picosulfate, aluminum hydroxide, calcium carbonate, magnesium carbonate, magnesium citrate, magnesium hydroxide, sodium zirconium cyclosilicate, trofinetide, cimetidine, dexlansoprazole, esomeprazole, famotidine, and pantoprazole.

This medication may not be suitable for people with certain conditions, so it is important to consult with a doctor if you have any health conditions. Some conditions that may need additional consideration include:

  • inflammatory bowel disease
  • undiagnosed abdominal pain, nausea, or vomiting
  • GI obstruction or perforation
  • ileus
  • toxic megacolon
  • rectal bleeding
  • appendicitis
  • gastroenteritis

As dizziness and fainting are side effects of bisacodyl, it is advised to not drive or operate machinery until it is clear how this medication affects you.

Some formulations of this medicine may contain benzyl alcohol.

Bisacodyl Tablets are not recommended for young children who are unable to swallow tablets.

This medication should not be used longer than 5 days. Consult a healthcare practitioner if constipation persists after 5 days of use.

Oral Bisacodyl Is Effective and Well-Tolerated in Patients With Chronic Constipation

This study included an examination of the efficacy and safety of 4 weeks of treatment with oral bisacodyl tablets in patients with chronic constipation.

It is a randomized, double-blind, placebo-controlled, parallel-group study conducted in 27 centers in the United Kingdom. Patients were separated into groups that were given 10mg bisacodyl (247 patients) or placebo (121 patients), once daily, for 4 weeks. Electronic diary each day helped them to record information relating to their constipation.

The mean (± standard error of the mean [SEM]) number of complete spontaneous bowel movements (CSBMs) per week during the treatment period increased from 1.1 ± 0.1 in both groups to 5.2 ± 0.3 in the bisacodyl group and 1.9 ± 0.3 in the placebo group (P < .0001). Parameters like the number of complete spontaneous bowel movements for every single week, number of spontaneous bowel movements (SBMs), and constipation-associated symptoms) showed improvements and bisacodyl efficacy (P < .0001).

The overall Patient Assessment of Constipation quality of life (PAC-QOL) score and all subscales (satisfaction, physical discomfort, psychosocial discomfort, worries, and concerns) were better in the bisacodyl-treated patients, compared with those that received placebo (P ≤ .0070).

The conclusion is that bisacodyl is an effective and well-tolerated treatment for patients with chronic constipation. This drug improves bowel function, constipation-related symptoms, and disease-related QOL.
